I have an extremely detailed yet flexible world to game on where I know, generally speaking at least, where everything is and what most everything does. I than drop the PCs on it and ask them where they want to go or what they want to do. I started drawing a simple map, have a basic concept in my head and let the actions of the party dictate what came next, but it's been a long time since I've had a chance to DM and I wanted there to be lots of content for them to explore. And I got what I deserved. Everyone seemed to pick it up quickly, which tells me it wasn't overly complex and everything seemed to be intuitive.
